Carrie Reedy Functional Nutrition provides a nutritional and lifestyle consultancy service. I, Carrie Reedy, am able to provide this service as a holder of a Bachelor of Health Science in Nutritional Medicine. Our service does not diagnose, prevent or claim to treat or cure any medical conditions. The intention of our service is to uncover contributing factors that may be influencing your current health concerns and symptoms, and to provide individualised recommendations for dietary and lifestyle habits you can implement in your own life.
The initial consultation will take place either in person or online. This session will involve a thorough discussion regarding your health history. Questions will be asked about your body systems such as the digestive system, reproductive system, respiratory system, circulatory system, endocrine system, nervous system and urinary system. I will also ask about your family history of disease, and discuss your diet and lifestyle. I will also need to know if you are allergic to any foods, drinks, or supplements, and any pharmaceutical medications, or nutritional supplements you are currently taking. The purpose of the in-depth initial consultation is to formulate a specific ‘rebuilding your health’ recommendation plan, and provide nutritional and lifestyle advice suitable for your individual needs. From time to time pathology testing may be recommended to gain further insight into your challenges. The ‘rebuilding your health’ plan will be presented back to you at the second consultation.
As a health professional, I will abide by my duty of confidentiality. All sensitive records are securely stored. Please see the privacy policy for situation when I may be legally required to disclose personal information.
Nutritional supplements may be recommended as part of our ‘rebuilding your health’ plan. These supplements are also not intended to diagnose, prevent, treat or cure any health or medical condition. Any product purchased for whatever reason which is used as a food supplement is done so at the discretion of the customer. It is not uncommon to be intolerant to certain grains, nuts or food groups. Certain supplements contain such ingredients. If you have or suspect you may have allergies or medical issues which may be affected by certain foods please advise me, or, after taking or using any of our products you find you may have or are experiencing side effects, you should promptly contact your healthcare provider. Also, do not exceed the prescribed dosage of supplemental nutrients. You are using and taking all products at your own risk. We accept no liability for any result, direct or indirect, of you taking any product and neither carriereedy.com or Carrie Reedy or any employees of either may be held responsible for any claim made against them.
You should also consult a medical practitioner before taking any nutritional supplements, especially if you are pregnant or nursing. If you wish to cease taking pharmaceutical medications for any reason, or cease conventional medical care, you should seek advice from your healthcare provider.
Functional nutrition is suitable for all ages and stages of life. Through the use of dietary and lifestyle measures the body can come into balance, allowing all body systems to work optimally, which will increase the likelihood of greater health and wellness. By agreeing to implement advice provided, and by taking prescribed nutritional supplements, you are doing so at your own risk. There may be unwanted side effects, or there may be no improvement to your symptoms. I am unable to guarantee results, but I will do my best to ensure you have the best possible chance of improving with my recommendations. As a Nutritional Medicine Practitioner, I will provide recommendations within my scope of practice. Should the situation arise that your case is deemed outside my scope of practice, I will refer you to an appropriate health professional to assist you.
The clinic is located at 14 The Haven, Canning Vale. There is ample parking in the driveway of the property. In addition to face to face consultations from the Canning Vale office, I offer online consultations for clients living in Australia, through Zoom. Should you require an online consultation, details of the Zoom web address will be provided to you by email as soon as possible after the booking.
Consultation appointment can be made through our online booking system at the top right-hand side of this website. You will receive a confirmation email once the booking is scheduled. You will also receive a reminder 24-hours before your consultation. Should you not receive the initial email confirmation, please contact the clinic by email at carrie@carriereedy.com or by phone on 0416 104 745.
As part of the intake process, I will forward an online health history form. Please complete the form and return it to me at least 2 days prior to your consultation. Should I not receive the health history form until the initial consultation, more of the 60-minute initial consultation will be required to prepare your functional nutrition matrix and health timeline, meaning an additional consultation will be required at a later stage should further health history questioning be required.

Payments for consultations can be made in advance, at the time of booking, through our booking link on the top right corner of this website, or in the clinic by a mobile EFTPOS facility. No record of customer credit card details will be retained by www.carriereedy.com. A receipt will be provided either at the conclusion of the consultation, or by email for you to submit to your private health fund. In the event the mobile EFTPOS card reader is not working, I will request that you arrange payment for the consultation via a direct bank transfer at the time of the consultation, or as soon as possible following the consultation.
If you need to cancel your consultation, please provide at last 24 hours’ notice, as this allows me to offer your time slot to others who may be needing a consultation. If 24 hours’ notice is not provided, a cancellation fee may be charged. This will be at the discretion of the clinic. I understand that life sometimes gets in the way of your best intentions. Please contact the clinic to see what alternative arrangements can be provided.
I do not offer a refund for change of mind on prepaid consultations, but clients have 6 months to redeem a prepaid consultation. There is also no refund on nutritional supplements, unless there is a fault or if a product has been tampered with. Under the National Health Act and Therapeutic Goods Act guidelines for sale and dispensing of medications, it is illegal for a practitioner to re-dispense medicine that has already been dispensed to another patient. I also do not offer refunds for consultation services already provided. If I am unable to attend our scheduled consultation for any reason, I will endeavor to give as much notice as possible, and will reschedule the appointment at a mutually beneficial time.
Occasionally during an online Zoom consultation there are connection issues, either at my end or at your end. Should this occur, I will attempt to establish phone contact and continue the consultation. Should this not be possible, I will contact you by email and we can reschedule the appointment.
My website provides facilities for members of the public to make comments. I request that you use respectful and acceptable behavior in your interactions on this site. I reserve the right to delete any rude or offensive comments, or anything else I deem unacceptable.
The best way to contact the clinic is by email or phone (including text messages), during office hours. I may not be able to take your phone call if I am consulting with other clients. Please leave a message, and I will endeavor to get back to you within 24 hours.

If you have any issues or complaints arising out of your use of this website, or with these Terms and Conditions, please contact carrie@carriereedy.com or phone 0416104745 so we can resolve the dispute or concern through negotiation and discussion.
If we are unable to resolve a dispute by negotiation and discussion within 14 days, the parties must proceed to mediation with the assistance of an accredited mediator who is independent of the parties. The mediator is to be appointed by agreement of the parties or, failing agreement within twenty-one (21) days of the first notification of the dispute, by a person appointed by the Chair of Resolution Institute, (ACN 008 651 232, Level 2, 13-15 Bridge Street, Sydney NSW 2000; telephone: 02 9251 3366, email: infoaus@resolution.institute) or the Chair’s designated representative. The Resolution Institute Mediation Rules shall apply to the mediation. We agree to share the costs of mediation equally between us.
It is a condition precedent to the right of either party to commence litigation other than the interlocutory relief that it has first offered to submit the dispute to mediation. Litigation is to be considered a last resort and may not be commended until, in the opinion of an independent mediator, the potential for negotiation and mediation have been exhausted.
